For individuals who are out of network or wish to pay Out-of-Pocket, a 20% reduction of the total billed amount may be applied if payment is received at the time services are rendered.

 

An itemized bill of services will be provided to the patient upon request for out-of-network or HSA/FSA submission. It is the patient's responsibility to submit to their third party carrier to obtain benefits or reimbursement.

 

Confirmation of Benefits: Third party payers have final determination regarding patient eligibility and benefits. Payment is due at time of service, including co-payments and deductibles if applicable.
